Пациенту в месте первичной госпитализации была проведена трансторакальная эхокардиография (ЭхоКГ). С целью дальнейшего дообследования и планирования тактики ведения больной был госпитализирован в ГБУЗ «Научно-исследовательский институт – Краевая клиническая больница № 1 им. проф. С.В. Очаповского» МЗ Краснодарского края, где обследование было дополнено компьютерной томографией (КТ) и КТ-ангиографией (КТА).</p></sec><sec><title>Результаты</title><p>Результаты: При проведении трансторакальной ЭхоКГ был выставлен диагноз: ВПС, ТФ, открытый артериальный проток, функционирующее овальное окно, правосторонняя дуга аорты и большие аорто-легочные коллатерали. При выполнении КТ и КТА было выявлено бивентрикулярное отхождение аорты, стенозы инфундибулярного отдела легочной артерии и выводного тракта правого желудочка, аномальное отхождение левой легочной артерии и правой подключичной артерии.</p></sec><sec><title>Обсуждение</title><p>Обсуждение: Трансторакальная ЭхоКГ является первоочередным скрининговым методом в поставке диагноза ТФ, однако ее данных недостаточно для планирования оперативного лечения. В качестве уточняющего метода необходимо выполнение КТА для детализации порока, объективной оценки аорты, легочных вен и артерий, а также для исключения экстракардиальной патологии.</p></sec><sec><title>Заключение</title><p>Заключение: Успех хирургической коррекции ВПС у детей заключается в предоперационной оценке всех анатомических особенностей, определяющих последовательность проводимого лечения и потенциальные хирургические риски. Использования одного метода визуализации для получения достоверных и надежных результатов при выборе хирургической тактики у пациентов со сложными ВПС недостаточно. КТА грудной клетки показала себя высокоэффективным методом в оценке порока и клинически важных сопутствующих анатомических особенностей.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: To demonstrate capabilities of multislice computed tomography (CT) in detailing a congenital heart disease (CHD), tetralogy of Fallot (TOF), in an infant.</p></sec><sec><title>Case report</title><p>Case report: We report a case of suspected TOF in a 3-month-old patient who underwent transthoracic echocardiography (TTE) at an initial hospitalization site and was admitted to the Scientific Research Institute – Ochapovsky Regional Clinical Hospital No. 1 (Krasnodar, Russian Federation) for further examination and treatment planning. The examination was supplemented with CT and computed tomographic angiography (CTA).</p></sec><sec><title>Results</title><p>Results: TTE diagnosed TOF, patent ductus arteriosus, functional foramen ovale, right-sided aortic arch, and major aortopulmonary collateral arteries. CT and CTA revealed biventricular origin of the aorta, infundibular pulmonary stenosis, right ventricular outflow tract stenosis, and abnormal origin of the left pulmonary artery and the right subclavian artery.</p></sec><sec><title>Discussion</title><p>Discussion: TTE is the primary screening method for TOF; however, its data are insufficient for planning surgical treatment. As a clarifying method, CTA is needed to detail the CHD, objectively assess the aorta, pulmonary veins and arteries, and exclude an extracardiac pathology.</p></sec><sec><title>Conclusions</title><p>Conclusions: The success of CHD surgical correction in children lies in the preoperative assessment of all anatomical features that determine the sequence of treatment and potential surgical risks. Using a single imaging method is insufficient to obtain reliable and accurate results when choosing a surgical strategy for patients with complex CHDs.
