Длительное консервативное лечение БС не имеет высокой эффективности, а хирургические вмешательства могут привести к повторным осложнениям. Клеточная терапия, признанная безопасным и эффективным методом стимуляции процессов регенерации, применяется во многих областях медицины, но в торакальной хирургии опыт ее применения ограничен.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: Оценить эффективность клеточного препарата, содержащего аутологичный лизат тромбоцитов и аллогенные фибробласты, для улучшения заживления БС и доказать безопасность его использования.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы: В исследование был включен 91 больной из торакальных отделений НИИ – ККБ № 1, пациенты были разделены на 2 группы: стандартная терапия (34 пациента) и клеточная терапия (57 пациентов).</p></sec><sec><title>Результаты</title><p>Результаты: В группе с применением клеточной терапии заживление бронхиальных свищей отмечено у 93% пациентов (без летальных исходов), в контрольной группе заживление свищей наступило только у половины пациентов, из них – у 59% после повторной операции, а общая летальность составила 35,3%. На протяжении всего периода наблюдения отмечалось отсутствие прогрессирования онкологического процесса у 47,4% пациентов основной группы и 20,6% – контрольной, что подтверждает безопасность и отсутствие онкогенности клеточного препарата.</p></sec><sec><title>Выводы</title><p>Выводы: Описанные результаты свидетельствуют об эффективности клеточного препарата в регенерации тканей по сравнению с контрольной группой. Полученные данные о безопасности и эффективности могут стать основой для будущих масштабных исследований с целью улучшения клинических результатов лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: Bronchial fistulas (BF) are a dangerous complication of thoracic surgery accompanied with a high mortality rate. Long-term conservative treatment of BF is ineffective, and surgery can lead to repeated complications. Although cell therapy is considered a safe and effective method for stimulating regeneration and has found its use in many fields of medicine, its application in thoracic surgery is limited.</p></sec><sec><title>Objective</title><p>Objective: To evaluate effectiveness of a cell product (autologous platelet lysate+allogeneic fibroblasts) in enhancing BF healing and to prove its safety.</p></sec><sec><title>Materials and methods</title><p>Materials and methods: Our study included 91 patients from thoracic units of the Scientific Research Institute – Ochapovsky Regional Clinical Hospital No. 1 (Krasnodar, Russian Federation) who were divided into 2 groupes: standard therapy (34 patients) and cell therapy (57 patients).</p></sec><sec><title>Results</title><p>Results: In the group receiving cell therapy, BFs healed in 93% of the patients, with no death cases. In the control group, BFs healed only in 50% of the patients (59% of them, after reoperation), and the overall mortality was 35.3%. During the follow-up, no cancer progression was observed in 47.4% of the main group patients and 20.6% of the controls, proving safety of the cell product and absence of an oncogenic effect.</p></sec><sec><title>Conclusions</title><p>Conclusions: Our findings indicate that the cell product is effective in tissue regeneration compared with the standard therapy.
