В условиях операционной пациент находится под воздействием различных анестезиологических и хирургических факторов, при этом гемодинамика пациента постоянно изменяется из-за хирургической травмы, кровопотери и других причин, поэтому вопрос о необходимости проведения инфузионной терапии в операционной остаётся наиболее острым.</p></sec><sec><title>Цель</title><p>Цель: Анализ различных методов определения волемического статуса пациента и прогнозирование его ответа на инфузионную терапию.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы: Поиск публикаций производился двумя независимыми исследователями в базах данных и электронных библиотеках PubMed (MEDLINE), Cochrane Library, Google Scholar, eLibrary. В результате отбора было выявлено 40 наиболее информативных публикаций. Эти публикации легли в основу данного обзора.</p></sec><sec><title>Обсуждение</title><p>Обсуждение: В обзоре описаны статические гемодинамические показатели, функциональные тесты (инфузионная мини-проба, тест с пассивным подъёмом ног и конечно-экспираторный окклюзионный тест) и динамические методы диагностики (вариабельность пульсового давления, систолического давления и ударного объёма, индекс растяжимости нижней полой вены, временной интеграл скорости кровотока в выносящем тракте левого желудочка, вариабельность пиковой скорости кровотока на сонной артерии при дыхании и скорректированное время потока на сонной артерии, индекс растяжимости внутренней ярёмной вены при дыхании). Многочисленные исследования доказали большую прогностическую ценность динамических методов по сравнению со статическими; однако на данный момент универсального метода диагностики не существует.</p></sec><sec><title>Выводы</title><p>Выводы: Поиск идеальной комбинации методов определения волемического статуса пациента и прогнозирования его ответа на инфузионную терапию является предметом дальнейших исследований.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: The annually increasing number of surgical procedures worldwide makes the issue of adequate perioperative fluid therapy controversial. In the operating room, patients are exposed to various anesthetic and surgical factors, while their hemodynamics constantly change due to surgical trauma, blood loss, and other factors. Therefore, the question of the necessity of intraoperative fluid therapy remains particularly critical.</p></sec><sec><title>Objective</title><p>Objective: To analyze various methods for assessing a patient’s volume status and predicting their response to fluid therapy.</p></sec><sec><title>Materials and methods</title><p>Materials and methods: The publication search was conducted independently by two researchers using databases and electronic libraries, including PubMed (MEDLINE), Cochrane Library, Google Scholar, and eLibrary. Following the selection process, the 40 most informative publications were identified, which formed the basis of this review.</p></sec><sec><title>Results</title><p>Results: This review describes static hemodynamic parameters, functional tests (mini-fluid challenge, passive leg raise, and end-expiratory occlusion test), and dynamic diagnostic methods (pulse pressure variation, systolic pressure variation and stroke volume variation, inferior vena cava collapsibility index, left ventricular outflow tract velocity time integral, respiratory variation of peak systolic velocity, corrected carotid flow time, and internal jugular vein distensibility index).
