Центральным звеном синдрома выступает инсулинорезистентность, приводящая к компенсаторной гиперинсулинемии. Последняя активирует симпатическую нервную систему и ренин-ангиотензин-альдостероновую систему, способствуя задержке натрия, эндотелиальной дисфункции, артериальной гипертензии и атеросклерозу. В почках гипергликемия запускает сигнальные пути PKC и JAK-STAT/NF-κB, а также накопление конечных продуктов гликирования (AGE), что вызывает воспаление, фиброз и прогрессирование диабетической нефропатии.</p></abstract><trans-abstract xml:lang="en"><p>This article presents a review of the pathogenesis of cardiovascular-kidney-metabolic syndrome – a multifactorial condition in which cardiovascular, renal, and metabolic disorders mutually aggravate each other. Insulin resistance serves as the central link of the syndrome, leading to compensatory hyperinsulinemia. The latter activates the sympathetic nervous system and the renin-angiotensin-aldosterone system, promoting sodium retention, endothelial dysfunction, arterial hypertension, and atherosclerosis.
