Оценка возможностей эндоваскулярных методов в лечении артериовенозных соустий области кавернозного синуса.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Оценены результаты эндоваскулярного лечения 22 пациентов с каротидно-кавернозными соустьями (ККС) в период с 2008 по 2018 г. Количество мужчин составило 13 (59%), женщин – 9 (41%). У 15 (68%) пациентов имелся травматический характер повреждения, у 7 (32%) пациентов заболевание носило спонтанный характер. У всех пациентов имелась характерная клиническая картина ККС, подтвержденная данными анамнеза, ангиографического исследования. После верификации диагноза всем пациентам была выполнена транскатетерная эмболизация патологического  сообщения при помощи отделяемых баллонов, микроспиралей, клеевых композиций. </p></sec><sec><title>Результаты</title><p>Результаты. 21 (95,5%) пациенту выполнена одноэтапная эмболизация ККС, одному пациенту (4,5%) потребовалось проведение 3 процедур (две эмболизации отделяемыми баллонами, финальная эмболизация отделяемыми спиралями) в связи с дефляцией баллонов в раннем послеоперационном периоде. Деструктивная эмболизация каротидно-кавернозного соустья выполнена в 2 (9%) случаях, реконструктивные вмешательства проведены 20 (91%) пациентам. Ангиографический успех процедуры составил 100%. У всех пролеченных пациентов уже в период нахождения в стационаре наблюдался регресс симптоматики. В течение госпитального периода было выявлено одно осложнение в виде постпункционной гематомы, потребовавшей консервативного лечения.</p></sec><sec><title>Заключение</title><p>Заключение. Эндоваскулярное лечение является современным, эффективным и безопасным методом лечения ККС. Деструктивная окклюзия (в случае отсутствия возможности выполнения реконструктивного вмешательства) также является безопасным и клинически оправданным методом лечения данной патологии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Assessment of endovascular methods possibilities in the treatment for carotid-cavernous fistulas.</p></sec><sec><title>Material and methods</title><p>Material and methods. The results of endovascular treatment in 22 patients with carotid-cavernous fistulas in the period from 2008 to 2018 were evaluated. The number of male patients was 13 (59%), female patients – 9 (41%). Fifteen patients (68%) had traumatic damage, 7 patients developed a spontaneous disease. All patients had a specific clinical features of carotid-cavernous fistula, confirmed by anamnesis and angiographic study. After verification of the diagnosis, all patients underwent permanent transcatheter embolization of the pathological connection using detachable balloons, microspirals and adhesive composition. </p></sec><sec><title>Results</title><p>Results. Twenty-one patients (95.5%) underwent one-stage embolization of carotid-cavernous fistulas. One patient (4.5%) required three procedures: two embolization with detachable balloons, final embolization with detachable spirals, due to deflation of the balloons in the early postoperative period. Destructive embolization of carotid-cavernous fistulas was performed in 2 cases (9%), reconstructive interventions were performed in 20 cases (91%). The angiographic success of the procedure was 100%. All treated patients had regression of symptoms during their hospital stay. During this period, one complication was identified in the form of post puncture hematoma, which required conservative treatment.</p></sec><sec><title>Conclusion</title><p>Conclusion. Endovascular treatment is a modern, effective and safe method for treating carotid-cavernous fistulas.
