Истории применения кондуитов в сердечно-сосудистой хирургии уже более 100 лет. С их использованием стала возможной помощь новорожденным и младенцам со сложными врожденными пороками сердца. Используются различные типы кондуитов, которые при сравнении имеют преимущества и недостатки, и в настоящее время трудно выделить «идеальный» кондуит. Основной проблемой всех типов кондуитов являются повторные операции, порой неоднократно на протяжении жизни пациента.</p></sec><sec><title>Цель</title><p>Цель. Представление непосредственных и среднесрочных результатов операций с использованием кондуитов в легочной позиции у детей до 1 года.</p></sec><sec><title>Методы</title><p>Методы. С января 2006 по июнь 2016 г. в нашей клинике 48 пациентам выполнена реконструкция выводного отдела правого желудочка с использованием различных кондуитов. Возраст пациентов составил от 14 дней до 1 года (4,82 ± 4,09 мес.), средняя масса тела 4,7 ± 2,3 кг (2,9–10,2 кг).</p></sec><sec><title>Результаты</title><p>Результаты. В ближайшем послеоперационном периоде умерли 7 пациентов (общая летальность 14,59%). У всех выписанных больных по данным ЭхоКС средняя скорость кровотока на кондуите составила 2,42 ± 0,72 м/с (от 1,3 до 3,9 м/с), а средний градиент 22,22 ± 17,17 мм рт. ст. (от 4 до 60 мм рт. ст.). Выполнено 18 повторных операций. Ранней реоперации (в течение 1 года после первичного вмешательства) потребовали 3 больных (8,82%), причиной в 2 случаях послужил стеноз кондуита, у 1 больного развилась аневризма правого желудочка. При использовании кондуита Contegra при среднем сроке наблюдения в 16,3 ± 11 мес. (1–62 мес.) повторные операции не требовались.</p></sec><sec><title>Заключение</title><p>Заключение. 1. Выбор кондуита для имплантации в легочную позицию продолжает оставаться серьезной проблемой при лечении сложных врожденных пороков сердца у новорожденных и младенцев.</p></sec><sec><title>2</title><p>2. При использовании кондуита Contegra при среднем сроке наблюдения в 16,3 ± 11 мес. (1–62 мес.) повторные операции не требовались, но для достоверных выводов требуется более длительный период наблюдения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. History of conduits application in cardiovascular surgery extends back for more than 100 years. With their use it became possible to help newborns and infants with difficult congenital heart diseases. Various types of conduits are used and well-known and have advantages and shortcomings, but it is difficult to choose an “ideal” conduit. The main problem of all types is repeated operations, which sometimes are repeated throughout the patient’s life.</p></sec><sec><title>Objective</title><p>Objective. Show immediate and medium term results with conduit application in pulmonary position in infants.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. From January 2006 to June, 2016 in our Clinic we have operated on 48 patients and performed reconstructions of the right ventricular outflow tract applying many different conduits. Age range was from 14 days to 1 year (4,82 ± 4,09 months), mean weight was 4,7 ± 2,3 kg (2,9-10,2 kg).</p></sec><sec><title>Results</title><p>Results. In immediate postoperative period 7 patients died (overall mortality rate was 14,59%). In all discharged patients according to ECG data mean blood flow rate on the conduit was 2,42 ± 0,72 m/s (from 1,3 to 3,9 m/s), and a mean gradient was 22,22 ± 17,17 mm Hg (from 4 to 60 mm Hg). We performed 18 repeated operations. Early reoperation was necessary in 3 cases(8,82%) (during the 1 year after the primary surgery), in cases the reason was conduit stenosis, in 1 patient there was a right ventricle aneurysm. We avoid repeated surgery in cases with conduit Contegra application with mean follow-up period 16,3 ± 11 months (1-62 months).</p></sec><sec><title>Conclusion</title><p>Conclusion. 1. Conduit selection for implantation in the pulmonary position has a tendency to be a serious challenge in infants and newborns treated for complicated congenital heart disorders.</p></sec><sec><title>2</title><p>2.
