При выполнении пероральной холангиоскопии особое внимание уделяется макроскопическим признакам поражения слизистой желчных протоков, в частности, патологическому строению капиллярного сосудистого рисунка, наличию грануляций и других разрастаний, пальпаторным характеристикам стенки, а также морфологической верификации при выполнении внутрипротоковой щипцовой биопсии под визуальным контролем.</p></sec><sec><title>Результаты</title><p>Результаты: Пероральные эндоскопические исследования ЖВП достоверно повышают эффективность дифференциальной диагностики различных видов протоковых стриктур. Возможность прямого осмотра слизистой протоков с оптически контролируемой щипцевой биопсией и морфологической верификацией позволяет увеличить чувствительность определения характера стриктуры до 83,3–96%, а специфичность до 90,9–99%. Диагностическая ценность пероральной холангиоскопии в диагностике злокачественных и доброкачественных поражений слизистой ЖВП превосходит эффективность применения эндоскопической ретроградной холангиографии, в том числе с выполнением внутрипротоковой верификации под рентгеноскопическим контролем.</p></sec><sec><title>Заключение</title><p>Заключение: Методика пероральной холангиоскопии занимает важную роль в лечебно-диагностическом алгоритме ведения пациентов с различными заболеваниями органов билиопанкреатодуоденальной зоны, в том числе с билиарными стриктурами, а также имеет широкие перспективы развития. На основании анализа литературных данных и собственного опыта применения методики пероральной холангиоскопии в группе больных с патологией ЖВП, в том числе при формировании недифференцированных билиарных стриктур, сформулированы основные показания к применению анализируемой методики. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: To determine the role of and indications for peroral cholangioscopy using the SpyGlass system in the differential diagnosis of biliary tract lesions and in case of biliary strictures, based on the literature data analysis and our own experience.</p></sec><sec><title>Materials and methods</title><p>Materials and methods: Peroral cholangioscopy is mainly used for the differential diagnosis of biliary tract lesions. During peroral cholangioscopy, we carefully consider gross signs of damage to the bile duct mucosa: abnormal capillary vascular pattern, granulation tissue and other types of proliferation, palpatory characteristics of the wall. Morphology should also be verified during image-guided intraductal forceps biopsy.</p></sec><sec><title>Results</title><p>Results: Peroral endoscopy of the biliary tract significantly increases the effectiveness of differential diagnosis between various types of biliary strictures.</p><p>Direct examination of the bile duct mucosa with optical forceps biopsy and morphological verification increases the sensitivity and specificity of stricture type determination up to 83.3%–96% and 90.9%–99%, respectively. The diagnostic value of peroral cholangioscopy in the diagnosis of malignant and benign biliary tract lesions exceeds the effectiveness of endoscopic retrograde cholangiopancreatography, even with fluoroscopy-guided verification of ducts.</p></sec><sec><title>Conclusions</title><p>Conclusions: Peroral cholangioscopy with its enormous potential plays an important role in management of patients with various diseases of the bilio-pancreatoduodenal area, including biliary strictures.
