Для увеличения эффективности специализированной помощи для данной категории пациентов, необходимо проведение исследований, направленных на оценку охвата населения оперативными вмешательствами.</p></sec><sec><title>Цель</title><p>Цель: Анализ выполненных реконструктивных нейрохирургических вмешательств в стационарах города Новосибирска.</p></sec><sec><title>Методы</title><p>Методы: Многоцентровое обсервационное ретроспективное нерандомизированное исследование проведено за период с 2016 по 2022 г. на базе 5 нейрохирургических отделений г. Новосибирска, выполняющих краниопластики. Из общего числа выполненных вмешательств (n = 673) 450 наблюдений соответствовали критериям включения.</p></sec><sec><title>Результаты</title><p>Результаты: Большинство пациентов (n = 361, 80,2%) было прооперировано в федеральных учреждениях. Среди больных, соответствующих критериям включения, преобладали городские жители (n = 260). Основной причиной выполнения краниоэктомий являлись последствия ЧМТ (n = 319, 70,9%). Сроки выполнения краниопластики варьировали от 0,1 до 320 мес., среднее значение – 23,9 ± 39,2, медиана – 11,6 мес. Значимые различия между первичной и реконструктивной операциями выявлены при сравнении последствий ЧМТ со всеми прочими патологиями, однако на практике они не имели клинической значимости. Указание двух характеристик, описывающих размеры и сложность дефекта, зафиксированы в 51,1% наблюдений. В 19,3% диагноз был сформулирован как «дефект костей черепа» с указанием анатомических областей.</p></sec><sec><title>Заключение</title><p>Заключение: За 7 лет в 5 нейрохирургических стационарах города краниопластика выполнена 450 жителям Новосибирска и Новосибирской области (64,3 операции в год). Сопоставляя полученные результаты с предыдущими исследованиями по количеству выполненных краниоэктомий, дефицит охвата реконструктивными вмешательствами составил 99,7 случаев в год. Для своевременного проведения краниопластик необходимо создание регистров больных с дефектами костей черепа.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: Studies on reconstructive neurosurgical interventions are always relevant due to the persistence of cranial defects, mainly caused by traumatic brain injuries (TBI). To provide medical care to the patients with cranial defects more efficiently, it is necessary to assess a large number of surgical interventions in studies.</p></sec><sec><title>Objective</title><p>Objective: To analyze data on reconstructive neurosurgical interventions performed at Novosibirsk hospitals.</p></sec><sec><title>Methods</title><p>Methods: We performed a multicenter observational retrospective non-randomized study of the total cranioplasty surgical interventions from 2016 to 2022. The study involved five neurosurgical departments in Novosibirsk hospitals. In 450 of 673 cases the inclusion criteria were met.</p></sec><sec><title>Results</title><p>Results: The majority of patients (n = 361; 80.2%) underwent surgical interventions in federal health facilities. Urban residents prevailed (n = 260) among the patients who met the inclusion criteria. Craniectomies were mainly indicated for the consequences of TBI (n = 319; 70.9%). Time to cranioplasty ranged from 0.1 to 320 months (mean: 23.9 ± 39.2; median: 11.6 months). There were non-clinically significant differences in terms of primary and reconstructive operations between the defects caused by TBI and other conditions. In 51.1% of cases, we found two features describing the size and complexity of the skull defect. In 19.3% of cases, patients were diagnosed with a skull defect (affected anatomical areas were specified).</p></sec><sec><title>Conclusions</title><p>Conclusions: Over seven years, 450 Novosibirsk residents underwent cranioplasty in five neurosurgical Novosibirsk hospitals (mean: 64.3 surgical interventions a year). Comparing the recent results with previous studies data in terms of performed craniectomies, we found that reconstructive interventions are lacking 99.7 cases per year.
