Одной из стратегий решения этой проблемы является разработка новых раневых покрытий (РП) с инкорпорированными антисептиками, которые могут предотвратить или снизить риск инфицирования ран без использования антибиотиков.</p></sec><sec><title>Цель</title><p>Цель: Провести сравнительную оценку эффективности РП на основе хитозана с иммобилизированными антисептиками и их комбинациями в эксперименте in vitro и in vivo.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы: Проведено многоэтапное рандомизированное контролируемое исследование in vitro и in vivo. Объектом исследования послужили образцы РП на основе хитозана с инкорпорированными антисептиками: полигексанидом, октенидина дигидрохлоридом, повидон-йодом и комбинацией полигексанида и октенидина в международных концентрациях и в разведении. Оценка противомикробной активности антисептиков in vitro проводилась микродилюционным и диско-диффузионным методами. В качестве тест-культур использовали культуры Staphylococcus aureus и Escherichia coli. Для исследований in vivo у лабораторных животных (крысы-самцы массой 300 ± 50 гр.) по собственной методике формировали экспериментальные гнойные раны мягких тканей с дальнейшим лечением путем применения исследуемых РП. Оценку динамики раневого процесса проводили путем анализа клинических и цитологических данных, а также с помощью прижизненной УЗ диагностики.</p></sec><sec><title>Результаты</title><p>Результаты: В зависимости от пролонгации сроков поддержания эффективной концентрации в ране и минимального раздражающего влияния на ткани, установлен следующий рейтинг антисептических средств: полигексанид&gt;октенидин&gt;повидон– йод. Комбинация полигексанида и октенидина не продемонстрировала искомый эффект синергизма, однако также оказывала положительное лечебное действие в ране, сопоставимое с действием отдельно взятых антисептиков. При комплексной оценке раневого процесса наилучший ранозаживляющий и антисептический эффекты продемонстрировали РП с иммобилизированным полигексанидом: достоверные признаки заживления раны наблюдались уже к 7-м сут. с дальнейшей положительной динамикой, что оказалось почти в 2 раза быстрее, чем при применении других опытных образцов.</p></sec><sec><title>Заключение</title><p>Заключение: Хитозан является перспективным материалом при использовании в качестве химически инертной матрицы для программируемой доставки в раневую среду лекарственных препаратов различного, в том числе антимикробного типа действия. Исследуемые РП с иммобилизованными антисептиками не проявили выраженного цитотоксического действия и угнетающего влияния на процессы ремоделирования мягких тканей, и, напротив, существенно сократили сроки перехода раны в пролиферативную фазу.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: A pressing issue in modern purulent surgery is the growing number of microbial strains resistant to antibiotics. One of the strategies to address this issue is to develop novel wound dressings (WD) with incorporation of antiseptics that can prevent or reduce infection without conventional antibiotics.</p></sec><sec><title>Objective</title><p>Objective: To compare the efficacy of chitosan-based WD with immobilized antiseptics and their combinations in in vitro and in vivo experiments.</p></sec><sec><title>Materials and methods</title><p>Materials and methods: We conducted a multistage randomized controlled in vitro and in vivo study investigating samples of chitosan-based WD with incorporated antiseptics (polyhexanide, octenidine dihydrochloride, povidone-iodine, and a combination of polyhexanide and octenidine) in international concentrations and diluted. The antimicrobial activity of antiseptics was evaluated in vitro by broth microdilution and disk diffusion methods. Staphylococcus aureus and Escherichia coli were used as reference cultures. For in vivo experiments, we formed experimental purulent wounds of soft tissues in laboratory animals (male rats weighing 300±50 g) according to our own method and treated the wounds with the investigated WD. The dynamics of the wound process was assessed by clinical and cytological data, as well as intravital ultrasonography findings.</p></sec><sec><title>Results</title><p>Results: Depending on the prolongation of time to maintain an effective concentration in the wound and minimal irritating effect on the tissue, we ranked antiseptic agents as follows: polyhexanide&gt;octenidine&gt;povidone-iodine. The combination of polyhexanide and octenidine did not demonstrate the expected synergistic effect; however, it had a positive therapeutic effect on the wound comparable to that of other antiseptics used alone. The comprehensive assessment of the wound process showed that WD with immobilized polyhexanide demonstrated the best wound healing and antiseptic effect. Reliable signs of wound healing with further positive dynamics were observed by day 7. It was almost 2 times faster compared with other experimental samples.</p></sec><sec><title>Conclusions</title><p>Conclusions: Chitosan is a promising material to be used as a chemically inert matrix for programmed delivery of various drugs, including antimicrobial drugs, into the wound.
