Протяженные поражения бедренного сегмента являются показанием к выполнению бедренно-подколенного шунтирования. При кальцинозе бедренных артерий возникают сложности при формировании проксимального анастомоза, в частности, при наложении сосудистого зажима на артерии. Помимо технических трудностей при формировании анастомоза, выраженный кальциноз увеличивает риски возникновения кровотечения, эмболических осложнений и влияет на длительность выполнения оперативного вмешательства. В случаях тотального кальциноза бедренных и подвздошных артерий использование баллонной окклюзии при формировании анастомоза позволяет окклюзировать артерии без наложения сосудистого зажима, что уменьшает риски периоперационных осложнений.</p><p>В данной статье представлен случай выполнения бедренно-подколенного шунтирования аутовеной пациенту с тотальным кальцинозом подвздошно-бедренного сегмента и окклюзией поверхностной бедренной артерии.</p></abstract><trans-abstract xml:lang="en"><p>Calcification of the lower extremities arteries is an unfavorable factor for vascular surgery. Extensive lesions of the femoral segment are an indication for femoropopliteal bypass surgery. Calcification of the femoral arteries complicates the construction of a proximal anastomosis because it precludes clamping the arteries. In addition to technical difficulties in the construction of an anastomosis, significant calcification increases the risks of bleeding and embolic complications and the duration of surgery. In cases of total calcification of the femoral and iliac arteries, balloon occlusion during the construction of an anastomosis makes it possible to occlude the arteries without clamping, which reduces the risks of perioperative complications.
