<trans-abstract xml:lang="en"><p>In this article we present a clinical case of successful treatment in a patient with acute coronary syndrome without ST-segment elevation with prolonged non-occlusive thrombosis of the anterior descending artery with means of thromboaspiration and distal protection. We analysed literature sources of a series of studies performed in patients with acute myocardial infarction. In the present clinical case with a formed thrombus in the anterior descending artery we failed to achieve optimal outcomes by performing thromboaspiration. Thrombus formed in a "clear" artery without stenosis was removed with a filter-trap.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>острый коронарный синдром</kwd><kwd>тромбоаспирация</kwd><kwd>тромбэкстракция</kwd></kwd-group><kwd-group xml:lang="en"><kwd>acute coronary syndrome</kwd><kwd>thromboaspiration</kwd><kwd>thrombextraction</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Sianos G., Papafaklis M.I., Serruys P.W.
